Dental Membership Plans
Hamby Family Dental Center offers an annual membership plans so you may save money on your dental care while maintaining your dental health. Our priority is to deliver quality care to informed patients in a comfortable and convenient setting.
Adult Preventative Dental Plan
Our adult preventative dental plan has a yearly membership fee of $495.00 and is also valid for one year from date of purchase. Any unused portion of the membership will be forfeited upon expiration.
The Adult Preventative plan entitles you to:
- 2 regular cleanings
- 2 exams
- 1 set of bitewing x-rays or panorex
- 1 emergency exam (with dental x-rays as needed)
- 15% discount on operative procedures
- Valued at $588
Child Preventative Dental Plan
Our child preventative dental plan has a yearly membership fee of $456.00 and is also valid for one year from date of purchase. Any unused portion of the membership will be forfeited upon expiration.
The Child Preventative plan entitles you to:
- 1 basic cleaning every 6 months
- Oral cancer screening with cleaning
- 1-2 exams/year
- X-rays as determined necessary
- 1 emergency exam with X-ray/year
- 1 Fluoride treatment/year
- Up to 10% off any needed treatment
- Valued at $602
If you cancel a membership for any reason, this also voids any discounts that you would receive for any treatment.
5% off with family members, 7% off with 3 family members, and 10% off with 4 or more family members.
